Social services

Our social services team seek to improve the quality of life and well being of each of our patients by helping them to cope with their medical health, emotional and social needs. They facilitate support groups, care conferences and provide counseling services for patients and their family members.

Dietary and nutritional services

At Kern River Transitional Care, we have a full-time registered dietitian on our team who works with patients with special dietary needs. Menu options are available within dietary and health care restrictions and our dietitian will work directly with our kitchen staff to make sure that healthy meals are provided to promote an improved lifestyle. When our residents have specific questions or concerns regarding dietary needs based on a current medical diagnosis or condition, our dietitian is readily available to assist them.

Health education

Health education increases individuals’ knowledge of health and health care and makes them informed about their health care choices. Our team aims to keep our patients’ lives as active as possible, working towards the goal of increased self-management behaviors and more independence.

Activities program

The mental and physical well being of each patient is incredibly important to us. Because of this, we offer an ongoing program, seven days a week to meet the interests of each patient through physical and mental activities. Our recreational team encourages a variety of activities which include: social gatherings, exercise classes, intellectual and spiritual discussions, arts and crafts classes, and an array of entertainment events. Our activity calendar changes monthly with new activities added accordingly including seasonal holiday events. We strive to provide interactive and fun activities where everyone can participate. Our activities department and nursing staff motivate residents by reminding, encouraging, and escorting them to the group activities of their choice.

Dental, Podiatry, Optometry, and Audiologist

All above services are available upon request and as ordered by your Attending Physician. While these services are offered by us, they are not covered under the room and board care. The provider will bill your insurance and/or give you an estimated cost if private pay.

Religious Services

A variety of religious services are available to our patients. Please talk to our activity director to discuss scheduled services.

Transportation services

Our team will assist in scheduling transportation service to outside appointments. The case management department will inform you if this service will be paid by your insurance or will need to be paid privately. If the outside appointment requires an attendant, the patient and/or family will be responsible in arranging someone to attend the appointment with the patient.
