Kern River Transitional Care offers qualified and experienced skilled nursing care to our patients in both a long-term and short-term care capacity. We have Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), as well as Certified Nurse Assistants (CNAs) available 24 hours a day to ensure that our resident’s skilled nursing needs are met.
Kern River has an experienced physical therapy team that treats health related conditions, illnesses, or injuries that affect the patients’ abilities to move and perform functional activities of daily living. Our team creates an individualized care plan based on an individual’s history and physical examination. Our physical therapy program includes muscle strengthening and retraining, transfer techniques, stair climbing, training in the use of prosthetics, gait training, and adaptive equipment training.
Occupational therapists (OTs) work with patients who suffer from physically, mentally, and/or emotionally disabling conditions by developing a care plan that improve basic motor functions and reasoning abilities. OTs provide customized interventions to improve individuals ability by providing treatment programs that may include: range of motion and strengthening exercises, functional ergonomics, and/or use of modalities to decrease pain and swelling. Occupational therapy focuses on the upper extremities to help our residents improve their ability to manage day-to-day living.
Our speech therapists provide treatments in the areas of speech, voice, fluency, and swallowing. Our speech and language therapy program is aimed at improving communicative abilities and cognitive functioning. We help our patients acquire reliable communication and swallowing skills that will fulfill their clinical need and social roles.
Kern River Transitional Care offers an extensive wound care program including: pressure, diabetic, vascular, traumatic, and post-operative. Our wound care team is also capable of replacing gastric and suprapubic tubes in order to further reduce unnecessary transfers. We have two specialized wound care nurses who work conduct weekly wound care rounds for patient consultation and follow up to ensure that wounds are properly cared for and promote maximum healing potential while our wound care surgeons regularly round several times per week.
Kern River Transitional Care provides pharmacy, diagnostic imaging and laboratory services on site ensuring that we have a rapid turnaround time on test results. We use the latest in laboratory testing and have an extensive quality assurance program for phlebotomy and laboratory operations.
From monitoring drug effectiveness to maintaining the right fluid balance our highly trained IV specialists and dedicated nurses work closely with our patient’s personal physicians to maximize comfort and speed recovery.
Our team of specialists work with our skilled nursing team to make you feel as comfortable as possible. They will work with you to make sure that you are provided the support needed upon first receiving your colostomy or ileostomy, as well as providing their knowledge to ensure that you are provided the correct guidance on how to care for your stoma.
Our nutrition team monitor patients’ weight, electrolytes, and other vitals along with fluid intake. Daily monitoring takes place to ensure that our patients’ daily energy requirements are met while emphasizing sterile techniques for catheter insertion to lower the risk of infection as best as possible.
